    Output-Only Modal Analysis Using Continuous-Scan Laser Doppler Vibrometry and Application to a 20kW Wind Turbine

    Continuous-scan laser Doppler vibrometry (CSLDV) is a method whereby one continuously sweeps the laser measurement point over a structure while measuring, in contrast to the conventional scanning LDV approach where the laser spot remains stationary while the response is collected at each point. The continuous-scan approach can greatly accelerate measurements, allowing one to capture spatially detailed mode shapes along a scan path in the same amount of time that is typically required to measure the response at a single point. The method is especially beneficial when testing large structures, such as wind turbines, whose natural frequencies are very low and hence require very long time records. Several CSLDV methods have been presented that employ harmonic excitation or impulse excitation, but no prior work has performed CSLDV with an unmeasured, broadband random input. This work extends CSLDV to that class of input, developing an output-only CSLDV method (OMA-CSLDV). This is accomplished by adapting a recently developed algorithm for linear time-periodic systems to the CSLDV measurements, which makes use of harmonic power spectra and the harmonic transfer function concept developed by Wereley. The proposed method is validated on a randomly excited free-free beam, where one-dimensional mode shapes are captured by scanning the laser along the length of the beam. The natural frequencies and mode shapes are extracted from the harmonic power spectrum of the vibrometer signal and show good agreement with the first seven analytically-derived modes of the beam. The method is then applied to identify the shapes of several modes of a 20kW wind turbine using a ground based laser and with only a light breeze providing excitation.

    Improved measurements of turbulence in the hot gaseous atmospheres of nearby giant elliptical galaxies

    We present significantly improved measurements of turbulent velocities in the hot gaseous haloes of nearby giant elliptical galaxies. Using deep XMM-Newton Reflection Grating Spectrometer (RGS) observations and a combination of resonance scattering and direct line broadening methods, we obtain well bounded constraints for 13 galaxies. Assuming that the turbulence is isotropic, we obtain a best-fitting mean 1D turbulent velocity of similar to 110 km s(-1). This implies a typical 3D Mach number similar to 0.45 and a typical non-thermal pressure contribution of similar to 6 per cent in the cores of nearby massive galaxies. The intrinsic scatter around these values is modest-consistent with zero, albeit with large statistical uncertainty-hinting at a common and quasi-continuous mechanism sourcing the velocity structure in these objects. Using conservative estimates of the spatial scales associated with the observed turbulent motions, we find that turbulent heating can be sufficient to offset radiative cooling in the inner regions of these galaxies (< 10 kpc, typically 2-3 kpc).     Deletion of parasite immune modulatory sequences combined with immune activating signals enhances vaccine mediated protection against filarial nematodes

    &lt;p&gt;Background: Filarial nematodes are tissue-dwelling parasites that can be killed by Th2-driven immune effectors, but that have evolved to withstand immune attack and establish chronic infections by suppressing host immunity. As a consequence, the efficacy of a vaccine against filariasis may depend on its capacity to counter parasite-driven immunomodulation.&lt;/p&gt; &lt;p&gt;Methodology and Principal Findings: We immunised mice with DNA plasmids expressing functionally-inactivated forms of two immunomodulatory molecules expressed by the filarial parasite Litomosoides sigmodontis: the abundant larval transcript-1 (LsALT) and cysteine protease inhibitor-2 (LsCPI). The mutant proteins enhanced antibody and cytokine responses to live parasite challenge, and led to more leukocyte recruitment to the site of infection than their native forms. The immune response was further enhanced when the antigens were targeted to dendritic cells using a single chain Fv-αDEC205 antibody and co-administered with plasmids that enhance T helper 2 immunity (IL-4) and antigen-presenting cell recruitment (Flt3L, MIP-1α). Mice immunised simultaneously against the mutated forms of LsALT and LsCPI eliminated adult parasites faster and consistently reduced peripheral microfilaraemia. A multifactorial analysis of the immune response revealed that protection was strongly correlated with the production of parasite-specific IgG1 and with the numbers of leukocytes present at the site of infection.&lt;/p&gt; &lt;p&gt;Conclusions: We have developed a successful strategy for DNA vaccination against a nematode infection that specifically targets parasite-driven immunosuppression while simultaneously enhancing Th2 immune responses and parasite antigen presentation by dendritic cells.&lt;/p&gt

    Hospital treatment -is it affordable? A structured cost analysis of vaginal deliveries and planned caesarean sections

    <p>Abstract</p> <p>Introduction</p> <p>The analysis of cost effectiveness in hospitals is as difficult as treating the patients properly. We are yet not able to answer the simple question of what costs are caused by a certain diagnosis and its treatment during an average hospital stay.</p> <p>Methods</p> <p>To answer some issues of the global problem of cost effectiveness during hospitalisation, we analysed the costs and the cost structure of a normal obstetrical hospital stay during an uncomplicated vaginal delivery and a planned caesarean section. Cost data was collected and summarized from the patients file, the hospital's computer system gathering all cost centres, known material expenses and expenses of non obstetrical medical services.</p> <p>Results</p> <p>For vaginal deliveries/planned caesareans we can calculate with a surplus of about 83 €/1432 €. About 45% of the summarized costs are calculated on a reliable database.</p> <p>Discussion</p> <p>The introduction of the DRG based clearing system in Germany has aggravated the discussion on cost effectiveness. Our meticulous work-up of expenses excluded personal precautionary costs and personnel costs of documentation because no tools are described to depict such costs. If we would add these costs to the known expenses of our study, we strongly suspect that hospital treatment of vaginal deliveries or planned caesarean sections is not cost effective.</p

    Endoscopic Saphenous harvesting with an Open CO2 System (ESOS) trial for coronary artery bypass grafting surgery: study protocol for a randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>In coronary artery bypass grafting surgery, arterial conduits are preferred because of more favourable long-term patency and outcome. Anyway <it>the greater saphenous vein </it>continues to be the most commonly used bypass conduit. <it>Minimally invasive endoscopic saphenous vein harvesting </it>is increasingly being investigated in order to reduce the morbidity associated with conventional open vein harvesting, includes postoperative leg wound complications, pain and patient satisfaction. However, to date the short and the long-term benefits of the endoscopic technique remain controversial. This study provides an interesting opportunity to address this gap in the literature.</p> <p>Methods/Design</p> <p><b>Endoscopic Saphenous harvesting with an Open CO<sub>2 </sub>System </b>trial includes two parallel vein harvesting arms in coronary artery bypass grafting surgery. It is an interventional, single centre, prospective, randomized, safety/efficacy, cost/effectiveness study, in adult patients with elective planned and first isolated coronary artery disease. A simple size of 100 patients for each arm will be required to achieve 80% statistical power, with a significant level of 0.05, for detecting most of the formulated hypotheses. A six-weeks leg wound complications rate was assumed to be 20% in the conventional arm and less of 4% in the endoscopic arm. Previously quoted studies suggest a first-year vein-graft failure rate of about 20% with an annual occlusion rate of 1% to 2% in the first six years, with practically no difference between the endoscopic and conventional approaches. Similarly, the results on event-free survival rates for the two arms have barely a 2-3% gap. Assuming a 10% drop-out rate and a 5% cross-over rate, the goal is to enrol 230 patients from a single Italian cardiac surgery centre.</p> <p>Discussion</p> <p>The goal of this prospective randomized trial is to compare and to test improvement in wound healing, quality of life, safety/efficacy, cost-effectiveness, short and long-term outcomes and vein-graft patency after endoscopic open CO<sub>2 </sub>harvesting system versus conventional vein harvesting.</p> <p>The expected results are of high clinical relevance and will show the safety/efficacy or non-inferiority of one treatment approach in terms of vein harvesting for coronary artery bypass grafting surgery.</p> <p>Trial registration</p> <p>www.clinicalTrials.gov <a href="http://www.clinicaltrials.gov/ct2/show/NCT01121341">NCT01121341</a>.</p
